Primary care reimagined: onsite, nearsite, virtual — and powered by Epic

The case for smarter primary care

Healthcare is at an inflection point. According to Mercer, A Marsh Business’s 2025 National Survey of Employer Sponsored Health Plans, the average cost of employer sponsored health insurance increased 6 percent and prescription drug coverage rose 9.4 percent in 20251, meaning employers are spending more and getting less.

Plus, members are navigating complexity without a guide and primary care is too often fragmented, reactive, and inaccessible.

QuadMed was founded on a different belief: that primary care works best when it is close, continuous, and connected. That belief has never been more relevant — and our approach has never been more advanced.

We deliver employer-sponsored primary care across onsite, nearsite, and virtual settings, integrated through a single clinical model and powered by Epic. The result is a care experience that is proactive rather than reactive, coordinated rather than fragmented, and measurably better for members, employers, and the health system as a whole.

Where we deliver care

QuadMed meets members wherever they are. Our care model is not defined by a single location — it is defined by a single standard of excellence, delivered flexibly across three complementary settings.

Onsite health centers

For employers with sufficient population density, onsite health centers are the gold standard. Members receive primary care, preventive services, chronic disease management, occupational health, and more, all without leaving campus. Proximity eliminates barriers. Familiarity builds trust. Outcomes improve.

Our onsite model is not a nurse’s office. It is a full-service primary care practice, staffed by board-certified providers, integrated with specialist networks, and designed to serve as a true medical home for the employees and dependents we serve.

Nearsite health centers

Shared nearsite health centers extend the QuadMed model to employers and populations that cannot support a dedicated onsite facility. Members access the same clinical quality and care coordination in a convenient, community-based setting without the overhead of a standalone clinic.

Nearsite centers are particularly powerful in multi-employer markets, where QuadMed operates as a convener — aggregating lives, sharing infrastructure, and creating access where none previously existed.

Virtual care

Virtual care is not a fallback. It is a feature. QuadMed’s virtual care platform is fully integrated with our clinical model, allowing members to see their own care team — not a random provider — via video, phone, or asynchronous messaging.

This continuity matters. When a member’s virtual visit is connected to their longitudinal health record, their care team can act on it. Virtual care at QuadMed is relational, not transactional.

For employers with distributed workforces, remote workers, or satellite populations, virtual care ensures that every employee has access to the same high-quality primary care experience.

Epic as a force multiplier

Technology is only as powerful as the clinical model it serves. At QuadMed, our investment in Epic — the leading electronic health record platform — is not a cost of doing business. It is a strategic differentiator.

One record. One story.

Every QuadMed patient has a single, longitudinal health record in Epic. Whether a member visits an onsite clinic, a nearsite center, or connects virtually, their care team sees the same complete picture: labs, medications, visit history, care gaps, chronic conditions, and more.

This is not incremental. In a fragmented healthcare system where the average patient’s record is scattered across dozens of disconnected systems, a unified record is transformational. Our providers make better decisions because they have better information.

Interoperability at scale

Epic’s interoperability capabilities allow QuadMed to exchange data with health systems, specialists, labs, pharmacies, and payers, ensuring that our care teams have the full clinical picture, and that transitions of care are seamless rather than disruptive.

When a QuadMed member sees a specialist or is hospitalized, our clinical team is notified. We close the loop. We follow up. We coordinate. This is the difference between a health center and a medical home.

Population health and analytics

Epic’s population health tools allow QuadMed to move from reactive care to proactive management. We don’t just identify members with uncontrolled chronic conditions before they become crises, we use predictive analytics to identify and target members at risk before conditions develop in the first place. We stratify risk. We close care gaps. We reach out.

For employers, this translates directly to outcomes: fewer emergency department visits, fewer hospitalizations, better management of high-cost conditions, and a workforce that is healthier, more productive, and more present.

Member experience

With Epic’s patient-facing platform, MyChart, QuadMed members get a seamless digital experience: online scheduling, secure messaging with their care team, access to test results, prescription renewals, and health education. Members are not passive recipients of care; they are active participants.

This engagement matters. Patients who are connected to their care team are more likely to follow through on preventive care, manage their chronic conditions effectively, and avoid unnecessary utilization later.

Our philosophy: supporting, not replacing, clinical judgment

AI and automation are reshaping healthcare administration — ambient documentation, in-basket management, prior authorization, coding support. These capabilities are real, and QuadMed is actively evaluating and deploying them where they reduce burden without introducing risk.

But our philosophy is clear: technology supports clinical decision-making; it does not replace it. Every care decision at QuadMed is made by a provider who knows their patient, informed by data, and accountable for outcomes.

Our Focus

Supporting, not supplanting, clinical judgment. Enhancing workflows without adding friction. Protecting member safety, trust, and data integrity.

This is not a conservative posture. It is a pragmatic one. We are building deliberately, deploying tools that make our providers more effective, not tools that create new categories of risk.

The employer value proposition

QuadMed exists because we believe employers can and should play a leading role in transforming healthcare for their workforce. The employer-sponsored care model is uniquely positioned to deliver what the traditional fee-for-service system cannot: access, continuity, accountability, and value.

Reducing total cost of care

When employees have a primary care home, they use it. And when they use primary care appropriately, they use expensive downstream care less. QuadMed’s clients consistently see reductions in the primary drivers of employer healthcare spend including emergency department utilization, specialist referrals, and avoidable hospitalizations.

Improving workforce health and productivity

Sick employees are less productive. Employees with unmanaged chronic conditions miss more work, are less engaged, and leave organizations at higher rates. QuadMed’s model addresses health at the source, providing the ongoing, relationship-based care that keeps people well and working, including addressing non-clinical factors like access, environment, and day-to-day barriers that come with traditional models.

Measurable, transparent outcomes

QuadMed provides employers with regular reporting on utilization, clinical quality, member satisfaction, and financial performance. We do not ask our clients to take our value on faith. We demonstrate it — in data, and in outcomes.

Moving forward — together

QuadMed is not just a vendor. We are a partner in the long-term health of your workforce. We bring clinical expertise, operational discipline, and technological capability to every engagement — and we measure our success by your outcomes.

The future of employer-sponsored primary care is integrated, data-driven, and human. Onsite. Nearsite. Virtual. Connected through Epic. Delivered by providers who know and care for your people over time.

We are scaling this model thoughtfully, guided by evidence, accountable to outcomes, and committed to the members and employers who trust us with their most important asset.
